Basic Life Insurance Plan
Coverage Amount: Equal to one times your annual salary ($15,000 minimum; $250,000 maximum)
Premium: Paid by the state
Optional Life Insurance Plan
Member: Starting with $15,000 up to six times salary (up to $800,000)
Premium Rate: Based on the age of the employee or retiree
Premium: Paid by the employee or retiree
Spouse: Starting with $15,000, up to $100,000 with Evidence of Insurability (cannot exceed coverage of the employee or retiree)
Premium Rate: Based on the age of the employee or retiree
Premium: Paid by the employee or retiree
Child(ren): $15,000 per child
Premium Rate: $1.50 per month
Premium: Paid by the employee, not available to a retiree
All life insurance is "term life" insurance. There is no cash or loan value. Benefits are paid upon the death of a covered participant.
